659 patient-form drafts in 18 categories and 345 contract drafts in 25 categories. They are mock drafts written as design references for a future forms engine.
C0520Are the forms legally valid templates?
No. Every draft is headed as a mock document for design reference, not legal advice, and requires counsel review before any use.
C0521Are the forms included in the product today?
No. The forms engine is planned. The library informs it and is not a shipped feature.
C0522Why build a library of contracts?
Clinics need business associate, data-use, vendor and exchange agreements. The drafts are a starting point for counsel and inform which templates a future engine should support.
C0523Are the contracts tied to US law?
They are grounded in common US health-sector arrangements. Latin American equivalents need local counsel and are not yet drafted.

The catalog of titles and categories is browsable on this site. The drafts themselves live in the project repository and are not offered as downloads.

The library has 96 consent drafts covering treatment, procedures and specialties. They are mock drafts.

37 drafts cover privacy practices and records release, and 33 cover notices and rights. All mock drafts.

87 drafts are screening and assessment instrument stand-ins. Licensed instruments need licences.

Not yet. Localized forms are a target and no translated drafts ship. A mock cross-border telehealth consent draft for Latin America exists only as a draft.

The forms engine and electronic signature are planned. Legal acceptance of electronic signatures differs by country and needs counsel.
