Notes, orders, lab and imaging

What exists Pilot
- Notes: progress, H&P, consult, procedure, discharge, telephone and nursing notes with templates and smart phrases, autosaved drafts, signing and co-signing, addenda, amendments and copy-forward.
- Vitals: structured entries with range flags, BMI, BSA, MAP and eGFR calculators, a NEWS2 score that can raise an alert, pain scales, intake and output, and trend charts.
- Orders: a catalog and order sets, priorities, signing by role per the permission matrix, verbal and telephone orders with read-back and a co-sign timer, and a duplicate and allergy check.
- Clinical inbox: results, messages, tasks and co-sign requests in one place, with assignment, delegation and escalation, letters and an after-visit summary.
- Forms: consents and forms with conditional logic, a typed or drawn signature, witness, versioning and hashed signed copies. 36 curated mock templates are bundled.
Simulated, and limits Simulated
- Lab and imaging: an HL7 v2 order and result codec behind a simulated connector, LOINC-coded results, a critical-result chain and simulated report ingestion. Nothing reaches an outside laboratory or radiology system.
- A drawn signature is not a certified electronic signature.
- Forms are mock drafts. They are not legal templates.
- LOINC is a curated list of 50 codes; SNOMED CT and CPT are not bundled.
- Only synthetic patients have ever been charted.
Common questions
Is the lab interface real?
No. It is a simulated connector that speaks HL7 v2 to a stand-in inside the hub. The parsing, the rejection of a result for the wrong patient and the critical-result chain are real code, but no outside system is connected.
Can a nurse sign an order?
Only as the permission matrix allows. Signing authority comes from the role class, is enforced on the server, and a verbal order needs a read-back and a co-signature.
